First, you must create or access your user profile by going to your provider’s program portal. You can do this by clicking the red "File a Claim/Service Request?" button above and selecting your provider from the drop down menu. Once you are on your provider's program portal, select "My Plan" and complete the needed information.

To view your billing information or to change your form of payment, log in to your personal account and then click on "My Billing."
